    Psychological model of ART adherence behaviors in persons living with HIV/AIDS in Mexico: a structural equation analysis

    OBJECTIVE The objective of this study has been to test the ability of variables of a psychological model to predict antiretroviral therapy medication adherence behavior. METHODS We have conducted a cross-sectional study among 172 persons living with HIV/AIDS (PLWHA), who completed four self-administered assessments: 1) the Psychological Variables and Adherence Behaviors Questionnaire, 2) the Stress-Related Situation Scale to assess the variable of Personality, 3) The Zung Depression Scale, and 4) the Duke-UNC Functional Social Support Questionnaire. Structural equation modeling was used to construct a model to predict medication adherence behaviors. RESULTS Out of all the participants, 141 (82%) have been considered 100% adherent to antiretroviral therapy. Structural equation modeling has confirmed the direct effect that personality (decision-making and tolerance of frustration) has on motives to behave, or act accordingly, which was in turn directly related to medication adherence behaviors. In addition, these behaviors have had a direct and significant effect on viral load, as well as an indirect effect on CD4 cell count. The final model demonstrates the congruence between theory and data (x2/df. = 1.480, goodness of fit index = 0.97, adjusted goodness of fit index = 0.94, comparative fit index = 0.98, root mean square error of approximation = 0.05), accounting for 55.7% of the variance. CONCLUSIONS The results of this study support our theoretical model as a conceptual framework for the prediction of medication adherence behaviors in persons living with HIV/AIDS. Implications for designing, implementing, and evaluating intervention programs based on the model are to be discussed

    Construction of the mutated NHE3 proton exchanger and his function

    Tema ovog rada bila je stvoriti mutirani transmembranski proteinski izmjenjivač Na+/H+ izooblik 3 (NHE3). Postupkom delecije uklonjene su 24 aminokiseline (Leu 586 – Arg609) važne za vezanje pojedinih stimulatora i inhibitora aktivnosti proteina NHE3 koji sudjeluje u održavanju homeostaze pH stanica. Delecija je provedena pomoću lančane reakcije polimerazom (PCR) metodom preklapanja dvaju fragmenta DNA sekvence gena NHE3. Predložak je bio plazmid pcDNA 3.1(+) s prethodno ugrađenom cDNA sekvencom gena HA-NHE3 (pcDNA/HA-NHE3). Uspješnost delecija provjerena je usporedbom dobivenog mutiranog fragmenta AD s kontrolnim fragmentom bez delecije. Fragment AD i plazmid pcDNA/HA-NHE3 tretirani su restrikcijskim enzimom PmaCI koji sekvencu NHE3 cijepa na dva mjesta. Pocijepani plazmid pcDNA/HA-NHE3 i fragment AD spojeni su ligazom i kao rezultat dobiven je plazmid s mutiranom sekvencom gena NHE3, pcDNA/dHA-NHE3. Produkt ligacije transformirao je bakterijske stanice Escherichia coli soja DH5α. Iz izraslih kolonija izoliran je plazmid pcDNA/dHA-NHE3 i pomoću restrikcijskih enzima HindIII i KpnI dokazana je uspješnost ligacije i pravilna orijentacija umetnutog fragmenta AD. Reakcijom PCR dokazana je uspješnost mutacije pomoću negativne početnice koja se veže za deletiran dio unutar gena NHE3. Konačna potvrda postojanja delecije je sekvenciranje dobivenog plazmida. Plazmidom pcDNA/dHA-NHE3 transficirane su stanične linije fibroblasta PS120. Postupkom zakiseljavanja medija dokazana je funkcionalnost proteina NHE3 unatoč mutaciji što bi značilo da unutar sekvence NHE3 postoji više regulacijskih mjesta aktivnosti proteina NHE3, zasebno ili u interakciji.The subject of this thesis was the construction of a mutated transmembrane isoform 3 Na+/H+ protein exchanger (NHE3). 24 amino acids deleted (Leu 586 – Arg609) are crucial for NHE3 protein regulation. The role of this exchanger is maintaining the homeostasis of cellular pH, by certain stimulators or inhibitors. The deletion has been accomplished by standard polymerase chain reaction method (PCR) using fragments of NHE3 sequence with flanking ends. The template for the reaction was pcDNA 3.1(+) plasmid with an inserted HA-NHE3 cDNA sequence (pcDNA/HA-NHE3). The efficacy of the deletion was checked by comparing the mutated fragment AD with control, unmutated fragment. After that, both pcDNA/HA-NHE3 plasmid and fragment AD were treated with PmaCI restriction enzyme having two restriction sites within the NHE3 sequence. Then, using the products of the reaction and treating them with ligase, a plasmid with a mutated NHE3 sequence was constructed, pcDNA/dHA-NHE3. Escherichia coli, DH5μ were transformed with ligation product and pcDNA/dHA-NHE3 plasmid was isolated from transformed colonies. The efficacy of the ligation and the proper orientation of the insert were examend by restriction analysis. The efficacy of the mutation was shown by PCR reaction using an anti-sense primer annealing to the deleted amino acid sequence. The final confirmation of the deletion was the sequencing of the isolated plasmid. A fibroblast PS120 cell line was transformed by pcDNA/dHA-NHE3 plasmid and the functionality of the NHE3 protein was shown by acid loading.
